AHEART will be hosting Community Day Appointments once per month. Clients will need to fill out a registration form for each month they wish to receive assistance. Clients should already have a referral form emailed to AHEART staff before they can pick up their items. Thursdays are the designated appointment days for each county. Every client will need to bring their baby to their appointment even if their child is sick. If sick, the infant can stay in the car, and an AHEART representative will meet you outside. AHEART organizará Citas del Día de la Comunidad una vez al mes. Los clientes deberán completar un formulario de registro por cada mes que deseen recibir asistencia. Los clientes ya deben tener un formulario de referencia enviado por correo electrónico al personal de AHEART antes de que puedan recoger sus artículos. Jueves son los días de cita designados para cada condado.
Cada cliente deberá llevar a su bebé a su cita incluso si su hijo está enfermo. Si está enfermo, el bebé puede quedarse en el automóvil y un representante de AHEART lo recibirá afuera. Rellene el siguiente formulario.
